Who we are
Premium Certus Lab Express, LLC is a mobile specimen collection service in Augusta, Georgia. This page covers what we collect through our website and careers portal: booking a visit, the visit request form and careers applications.

What we collect and why
- Booking a visit: your name, phone and email; the service and time you choose; for a visit to you, the address and any access notes; your answers to the service's questions, notes, and any documents you upload, such as a lab order or a photo ID. We use them to book, carry out and follow up on your visit.
- Payment: you enter your card in our payment provider's secure card form, and the provider handles the payment. We never see or store your card number; we keep only the card type and its last four digits.
- Visit requests: your name, phone, email if you give it, your company, the service, where and when, how many people and your notes, so we can call you back with a quote and a time.
- Careers applications: your name, phone, email, address and area; your answers about the role, experience, school, military service, driving, smartphone, skills and certifications; your CV and any certificates. Your legal name and date of birth are for the background check you authorize on the form. We never ask for a Social Security number.
- Careers portal sign-in with Google: when you sign in, Google gives us your Google account's identifier, your name and your email address (the
openid,emailandprofilepermissions, nothing more). We never see your Google password, and we keep no Google access token after you sign in. - Problems with the booking page: if a booking cannot go through, or you press "Report a problem", we keep what you had entered, your words, and technical details such as your browser, screen size and the errors the page showed, so we can fix it.
Who sees it
Our staff see your information when their work needs it. We do not sell your personal information.
How long we keep it
We remove information on a regular schedule. You can ask for your booking information at any time, and ask us to remove it, by writing to [email protected].
Cookies
This website sets no cookies of its own and has no advertising or analytics trackers. While you fill in a form, the page keeps a few things in your browser tab, such as the files you added, so a reload does not lose them; they go when you close the tab. The careers portal uses cookies only to keep you signed in and to protect its forms. A bot check runs on our forms to tell people from automated programs.
Your choices
Write to [email protected] to ask to see, correct or delete your information. You can close your careers portal account from the portal: that signs you out everywhere and removes your profile and unfinished applications. Applications you already sent are removed on our regular schedule.
Changes
When we change this policy, we update this page and the date at the top.
